Major Step Toward Commercialization with FDA

In a significant development for the field of wound care technology, Spectral AI (NASDAQ: MDAI), a Dallas-based leader in predictive artificial intelligence, has announced the formal submission of its innovative DeepView® System to the U.S. Food and Drug Administration (FDA). This regulatory milestone follows the triumphant completion and analysis of the company’s pivotal Burn Validation Study, the results of which powerfully demonstrate the system's superior accuracy in predicting burn wound healing potential.

The comprehensive Burn Validation Study, which commenced in January 2024 and concluded in March 2025, stands as one of the largest and most rigorous clinical trials for burn assessment ever conducted within the United States. The study enrolled a diverse cohort of 164 patients, comprising 115 adults and 49 pediatric patients, across multiple burn centers and emergency departments nationwide. The primary objective was to rigorously validate the DeepView System's ability to provide an immediate, objective, and highly accurate prediction of a burn wound's healing trajectory on the very first day of injury, thereby outperforming the current, often subjective, standard of clinical assessment.

The DeepView System itself is a sophisticated, non-invasive, cart-based device designed for exceptional mobility within a hospital setting. Its core technology leverages advanced multi-spectral imaging, capturing light across a range of wavelengths invisible to the human eye. This data is then processed by a proprietary AI algorithm, which has been trained and tested against an unparalleled proprietary database containing more than 340 billion clinically validated data points. The result is a predictive diagnostic tool intended to arm clinicians with an immediate and objective assessment of a burn wound's healing potential, guiding crucial treatment decisions like the need for surgery, before any medical intervention is undertaken.

The study's results were striking, showcasing the DeepView System's profound advantage over the clinical judgment of experienced burn physicians. The performance was meticulously assessed using several key metrics, comparing the AI's analysis against both physician assessments and the actual healing outcomes (the "ground truth").

Superior Sensitivity in Identifying Non-Healing Tissue: The system demonstrated a dramatic and statistically significant improvement in correctly identifying tissue that would ultimately fail to heal, a critical factor in deciding whether to perform a skin graft.

At the image-wise level (assessing the wound as a whole), the DeepView System achieved a sensitivity score of 86.6%, dwarfing the 40.8% score achieved by the clinical judgment annotation (CJA) of burn physicians.

This precision was even more pronounced at the pixel-wise level (mapping specific areas within the wound), where DeepView scored 81.9% compared to the physicians' 38.8% .

Exceptional Accuracy in Wound Mapping (Dice Score): The Dice Score, a measure of spatial overlap between the predicted area of non-healing tissue and the actual outcome, further underscored DeepView's precision. The system's pixel-wise evaluation yielded a score of 68.5%, significantly outperforming the physicians' score of 39.2%. This indicates the AI's far greater ability to accurately map the precise boundaries of tissue that will not heal.

Validated Specificity Performance: While physicians trended toward a more conservative assessment (overestimating the non-healing area), resulting in a high specificity score of 79.1% , the DeepView System's specificity for segmenting non-healing areas was a robust 61.2%. This result significantly exceeded the pre-trial anticipated goal of 36.0% , confirming the system's balanced and validated performance.

"This level of performance exceeds our highest expectations," stated Dr. J. Michael DiMaio, M.D., a key figure at Spectral AI. "The DeepView System has proven its ability to provide a level of predictive insight that is simply not achievable with the human eye alone. We are immensely grateful to the patients and clinicians who participated, and to our dedicated team led by Principal Investigator Dr. Jeffrey C. Carter. Our hope is that, following the FDA's thorough review and authorization, this groundbreaking tool will become an indispensable asset, enabling clinicians to make faster, more confident decisions, expedite patient care, reduce the need for multiple surgeries, and ultimately lower the overall cost burden on the healthcare system."

With the successful study results in hand, Spectral AI is moving forward with a clear regulatory and commercialization pathway. The Company plans to formally submit the complete data package to the FDA as part of its regulatory submission by the end of the second quarter of 2025. The goal is to secure FDA clearance through a De Novo Classification Request for the DeepView System in early 2026. Crucially, Spectral AI has already established its manufacturing arrangements and confirms it will be fully prepared to launch the DeepView System commercially in the United States immediately upon receiving final approval.
